Indiana residents completing this form please note:

Registration Deadline — 29 days before the election.

Form Box 6 – ID Number: Your state voter ID number is your ten digit Indiana issued driver’s license number. If you do not possess an Indiana driver’s license then provide the last four digits of your social security number. Please indicate which number was provided. (Indiana Code 3-7-13-13)

Form Box  7 – Choice of Party: Leave blank.

Form Box 8 – Race or Ethnic Group: Leave blank.

Form Box 9 – Signature: To register in Indiana you must:

• be a citizen of the United States
• have resided in the precinct at least 30 days before the next election
• be at least 18 years of age on the day of the next general election
• not currently be in jail for a criminal conviction

STEP 1: REGISTER TO VOTE

STEP 2: VERIFY YOUR VOTER REGISTRATION

Once your voter registration application is received, the county voter registration office will process the application and determine your eligibility. If you are eligible, the county office will send you a voter registration acknowledgment card. Once you receive that card, you will be officially registered.

If you are not eligible, or if the application was incomplete, you will receive a notice denying the application or requesting the missing information. If you have not received a voter registration card or a notice from your county within about 30 days, call the county voter registration office. You can check your voter registration and polling location at anytime at www.indianavoters.com or by calling 1-866-IN-1-VOTE.

STEP 3: MAKE SURE YOU’RE ELIGIBLE TO VOTE BY ABSENTEE BALLOT

All registered voters in Indiana are eligible to vote absentee-in-person at the county election board office beginning 29 days before Election Day.

In order to vote absentee-by-mail, one of the following must apply:

• You have a specific, reasonable expectation that you will be absent from the county on Election Day during the entire 12 hours that the polls are open (6 am until 6 pm).
• You have a disability.
• You are at least 65 years of age.
• You will have official election duties outside of your voting precinct.
• You are scheduled to work at your regular place of employment during the entire 12 hours that the polls are open.
• You will be confined due to illness or injury or you will be caring for an individual confined due to illness or injury during the entire 12 hours that the polls are open.
• You are prevented from voting because of a religious discipline or religious holiday during the entire 12 hours that the polls are open.
• You are a participant in the state’s address confidentiality program.
• You are a member of the military or a public safety officer.

Note:Voters voting absentee-by-mail are NOT required to show photo ID.

In order to vote absentee-by-traveling board, one of the following must apply:

• The voter expects to be confined, due to illness or injury, or the voter expects to be caring for a confined person at a private residence, on Election Day.
• The voter is a voter with disabilities and believes their polling place is not accessible to them.
• The ballot will be delivered to you by a bi-partisan absentee voter oard who will be able to assist you with you ballot.
• Voters voting by traveling board are NOT required to show photo ID.

Complete and sign the form, and mail it to the county Elections Office, 2293 North Main Street, Suite 16 Crown Point, IN 46307 (219) 755-3795.

STEP 5: RECEIVE, COMPLETE AND RETURN YOUR ABSENTEE BALLOT

The county Election Office will mail your absentee ballot and a return envelope.  Complete the absentee ballot, sign where indicated, and send it back to the county Election Office, 2293 North Main Street, Suite 16 Crown Point, IN 46307 (219) 755-3795.  Your voted absentee ballot must be received by the close of polls on election day.
